Long Term Care by Revera offers a warm, safe community to meet your care needs. In general, the admissions process for long term care is managed by a local long term care home placement agency (two examples are – Community Care Access Centres, CCACs, in Ontario; or Regional Health Authorities in most other Canadian provinces).

The agency works with you to assess your needs and to identify the options and choices available to you.

Based on this assessment, and if you are found eligible for long term care services, the agency will manage the placement process – everything from providing you with information about long term care homes in your area, to ensuring your name is on the appropriate waiting lists, to forwarding your application to the homes of your choice.
